The Centers for Medicaid and Medicare Services (CMS) has a process and team in place: the MDS (Minimum Data Sets)/IDT (Interdisciplinary Team) to assess each resident’s cares from the start of their stay and then ongoing once they make our communities their home. CDM, CFPPs, as the day-to-day leaders of the nutrition team, play an active role in both the process and the team. This educational session will highlight these critical elements to being successful in capturing personalized nutritional clinical plans of care for your residents.
