Long-term care for the 20th century has its mission to create a care environment for our seniors that will promote quality of life and quality of care mutually. The residents’ abilities to achieve successful nutritional outcomes is dependent on their personalized plan of care. The MDS 3.0 is the instrument used to ensure a true “head-to-toe” approach to individualizing their cares. Process driven, the RAI’s interdisciplinary approach defines the residents’ unique path towards achieving /maintaining their highest practical level of well-being.
